Federal Trade Commission developed a standardized form to assist consumers who are victims of identity theft. The FTC also offers excellent information to educate consumers about how to avoid becoming identity theft victims.
The FTC also provides links to other resources on identity theft and compromised financial accounts including the three major credit bureaus that you should report the problems to immediately.
The FTC Identity Theft Hotline (toll-free) (877) IDTHEFT (438-4338) provides advice on how to deal with credit related problems resulting from identity theft.
